Lisa Keefauver, MSW Reimagining Grief

Lisa Keefauver, MSW is a social worker turned grief and empathy activist; using her personal and professional experiences with grief and loss over the past 20 years to create a grief-smart culture. As founder of Reimagining Grief and Host of the podcast, Grief is a Sneaky Bitch, she is transforming and expanding our understanding of grief, and changing the lives of individuals, communities and organizations along the way. Lisa uses her warmth, vulnerability, humor and therapeutic skills in her work as an inspirational speaker, writer, podcast host, grief guide and educator. She even created a unique line of Empathy Cards to help us all find language when we’re at a loss for words.


Kizzy Chambers, LCSW, CMC RR Care Management, LLC

Kizzy Chambers MSW, LCSW, CMC is the Founder and Owner of RR Care Management, LLC in Orlando, FL. She is a Certified Care Manager and a Licensed Clinical Social Worker. Kizzy has over 15 years of experience in health care, working in hospital settings, and as a Care Manager. She is also a mental health therapist, focusing on depression, anxiety, stress management, and crisis stabilization. She primarily works with older and disabled adults. Kizzy attended the University of Central Florida, where she obtained her bachelor's degree in Psychology and her master's degree in Social Work. She is active with the Aging Life Care Association (ALCA) and currently serves on the ALCA Florida Chapter's board of directors. She also serves as the Orlando Unit Leader for the ALCA Florida Chapter. Kizzy served on the 2020 National ALCA Conference Committee. She was awarded the 2020 Outstanding Florida Chapter Member Award for her contributions and commitment to the profession and the association (ALCA). She is a member of the National Association of Social Workers (NASW) and the Association of Black Psychologists-Central Florida Chapter. She has written several articles, one which was published on the American Society on Aging blog. Kizzy enjoys assisting elder adults, and their family members navigate the complex medical system and resources, advocating for continuity of care, self-preservation, and dignity for elder and disabled adults. With her knowledge and experience as a Medical Social Worker, she understands the importance of working within a family system and mitigating family conflict.


John Steele Alternative Living Choices

John Steele created Alternative Living Choices after witnessing countless families become overwhelmed, lost and stressed when looking for a good home for their loved ones. John realized that after understanding their needs, he can do the work for them and present them with great options. Working closely with families, he reminds them that they are always the final decision maker.


Vic Holmes EDUCARE Ambassador

Vic Holmes is a 22+ year honorably retired Air Force Physician’s Assistant currently providing home-based medical care for the mentally and physically challenged. He also teaches medicine at the UNT Health Science Center in Fort Worth. Inspired by the injustice he and his then-partner-now-husband, Mark Phariss, endured during “Don’t Ask, Don’t Tell”, they became the first of the two plaintiff couples in the ultimately victorious Texas Ban on Same Sex Marriage lawsuit. They have supported LGBT equality in multiple arenas over the past two decades including the Human Rights Campaign, Equality Texas, and Ed-U-Care. He is also the Faculty Advisor for the amazing LGBT & Allies Student Group on the UNTHSC Campus. Most recently, he helped plan the 2016 CFA LGBT Summit in their hometown of Plano where their adorable rescue beagles bay incessantly.


Vivienne Armstrong EDUCARE

Vivienne is an RN with Master of Science in Community Health Nursing from Texas Woman's University and 40 years of experience in a variety of settings, including acute care, nursing education, community health, home health and hospice care.​ Member, American Nurses Association and Texas Nurses Association. Served on various city, county and state community health-related boards, commissions and committees to improve services for the elderly and those with HIV. Recognized with many prestigious awards for professional development and community service achievements. As residents of Dallas, Vivienne and her spouse, Louise Young, Ph.D. have been actively involved in the area’s social and political life for the past 40 years.​ Now as a retiree, Vivienne is committed to improvement of the health care and services for older adults including the Lesbian, Gay, Bisexual and Transgender (LGBT) community.


Jane Paccione Collective Impact at the San Antonio Area Foundation

Jane Paccione is the Managing Director of Collective Impact at the San Antonio Area Foundation. She previously served as the President and CEO of Ride Connect Texas, a local non-profit organization focused on increasing the independence of older adults and people with disabilities by providing them with mobility services. Additionally, Jane has owned and operated her own small business, leading her to receive the Texas Governor's Entrepreneurship Award. Jane has been part of the Successfully Aging and Living in San Antonio initiative since its inception, and as a result, understands the current challenges and opportunities to move the collaborative initiative forward. She has provided leadership on the Steering Committee as well as the Transportation workgroup. Due to Jane’s strong sense of community and her desire to help improve the lives of others, she has been actively involved and served in leadership roles with several organizations. Jane is a disability advocate and has educated others through several inclusion presentations for non-profit and for-profit businesses and organizations. Jane received a Master of Public Administration degree and a Bachelor of Arts degree in Sociology from The University of Texas at San Antonio.
